SUBACUTE CARE/SHORT TERM REHAB

At Ivy Hill, we offer skilled short-term rehab to assist those who, after a hospital stay, are in need of a sound rehabilitation program before returning home. Our comprehensive approach to nursing and rehabilitation care and support services provides quality, 24-hour nursing care while preserving every resident’s dignity and respecting their wishes. This approach is designed to support the transition from hospitalization to home, as quickly and as safely as possible. 

Each rehabilitation program is custom tailored to meet the unique needs of the individual with the goal of restoring function and help you reach the highest level of independence. Whether one needs to recover from recent surgery or to regain strength after an illness, our personalized rehab-to-home program at Harbour View will help you achieve your goal.

Physical, occupational and speech therapies aim to restore quality of life through independence and self-esteem. Therapeutic and specialized dietary services, plus three nutritious meals a day and snacks are also provided, along with choices in social and recreational programming and activities.  It is important that family members are regularly updated about their loved one’s wellness. We believe family counseling is an integral component of providing senior care, and we offer family members information, options and support in a sympathetic environment. Our combination of clinical expertise, state-of-the-art equipment, and dedicated and caring staff, will ensure that each resident returns home as soon as possible.
